The Bladder Care Program

The Bladder Care Program is supervised by a Registered Nurse specialized in the treatment of bladder health including assessment, exercises and training to promote healthy bladder care.

The Medication Administration Program

The Medication Administration Program involves the use of specific orientation techniques that educate about medications. We discuss your current medications, the medication schedule and how often each should be taken.

The Wound Care Program

The Wound Care Program has been designed to provide favorable clinical outcomes to our patients. Our Wound Care certified Nurse works work with your physician to develop and implement treatment protocols in wound healing techniques and care. We coordinate the patients overall care with physician and report wound progress weekly by providing wound measurements as well as other vital reports regarding patient’s progress.

The Memory Care Program

The Memory Care Program is designed for people with Alzheimer’s disease or Dementia. We employ innovative techniques and approaches for effective communication, and formulate strategies and interventions for memory recall. Our team is trained to value each senior and to recognize their different preferences and needs. We know that memory loss affects every person differently and focus on understanding each individual’s preferences.

The COPD (Chronic Obstructive Pulmonary Disease) Management

The COPD (Chronic Obstructive Pulmonary Disease) Management is designed to help patients manage their symptoms of COPD. It involves assessment and monitoring of the disease, reduction of risk factors, management of the COPD and management of exacerbations. We promote education, treatments and lifestyle changes that can help patients with chronic bronchitis, emphysema, chronic asthma, lung cancer, pneumonia, tuberculosis and other lung diseases.

The Cancer Care Program

The Cancer Care Program is designed for people coping with this unpredictable and potentially life­threatening illness and those who love and care for you. Our Program is designed to help alleviate the stress associated with caring for cancer patients, allowing the family to focus on the complex emotional needs you may have as you learn to deal with new challenges. Our Cancer Care Team is supervised by a Registered Nurse specialized in the treatment of Cancer symptoms who works with your physician, surgeon and oncologist to develop and implement an individualized treatment plan for you, based on the severity of your illness. The program involves a full nursing assessment and teaching about management of your symptoms. We provide Home Infusion Therapy, including the treatment and management of central lines and ports; Continuing care and education associated with chemotherapy and radiation and infusion administration; we also provide Wound care, Ostomy and continence care. Our caregivers also provide assistance with the activities of daily living.

Congestive Heart Failure Program

Congestive Heart Failure Program is a Proactive Disease Management program with emphasis on disease processes, symptom management and behavior modification. A full nursing assessment of the patient and the home environment is conducted. CHF Weight Guidelines carefully reviewed and documented with the patient and their caregivers. All CHF/COPD patients participate in our CHF protocol which focuses on physical status (such as weight, edema status, oxygen use, etc.) weight management, behavior modification and teaching. The Treatment Plans are reviewed with the patient and their families and emphasize the importance of communication with the patient as well as the physician, pharmacist and caregivers.

The Diabetes Management Program

The Diabetes Management Program includes education about diabetes, meal planning, medications, exercise, stress management, monitoring and preventing complications and setting goals. Behaviors modifications are taught which include healthy eating, being active, monitoring blood sugar, taking medication, problem solving, reducing risks and healthy coping.
